Patients with CD should avoid gluten and maintain a balanced diet to ensure an adequate intake of nutrients, vitamins, fibre and calcium. The practicalities of this however, are difficult, as i many processed foods are contaminated with gluten, ii staple GF foods are not widely available, and iii the GF substitutes are often expensive. Furthermore, iv the restrictions of the diet may adversely affect social interactions and quality of life.
The inclusion of oats and wheat starch in the diet remains controversial. The transition from a regular gluten-containing diet to a GFD is the first therapeutic intervention towards improved health after diagnosis of CD. At present, a strict GFD must be considered a lifelong treatment and patients should be seen by a dietitian at the time of diagnosis and offered follow-up to provide ongoing assessment, knowledge update and support.
Inclusion of gluten-free GF oats and wheat starch-based products has been controversial over the years, and is discussed in detail below.
